Once the site has finished loading, click on the three-dot menu on the top right of the Chrome browser and hover over the More tools and select create shortcut ...

Clicking create shortcut ... will prompt you to enter the name you want to create a shortcut in a popup window. So click on the name you want.

Clicking on Create will take you to the Home screen of your computer.
